How to order exams and materials:
Orders for exams and training materials can be submitted via email, fax, phone and mail Email orders to examorders@prometric.com., fax to 800-347-9724, phone to 800-280-3926 or mail to Prometric, Food Safety, 1260 Energy Lane, St Paul, MN 55108. Please include the Materials and Exam (CPFM/HACCP) Order Form and your payment information. Training materials and all shipping and handling must be paid for at time of order. Payment Options:
If you wish to be invoiced, we will accept a purchase order (PO) provided that you are ordering more than 10 exams. The PO must be on company letter head and included with your completed (CPFM/HACCP) materials and exam order form. Please make sure it is specific -- the billing address, the company address, the quantity and type of exams or materials ordered and any shipping charges. We cannot invoice based on a PO number alone. Using a PO will ensure more accurate orders and invoicing. It is for your protection, in the unlikely event of a discrepancy with the invoice.
If you are ordering fewer than 10 exams you must include your payment with a completed (CPFM/HACCP) materials and exam order form. We accept Visa, MasterCard, American Express, money order, cashier's check or company check for any order quantity.
Quantity of exams to order:
Please order the number that you know you will use. We understand that at times you order extra to accommodate last-minute candidates. Exams change every three months; however, they are valid for a year. If you find you have too many exams, they can be used with your next class. Please be sure to use your oldest exams first. Refunds and returns will not be issued for unused exams.
